Make an occupational therapy referral

For participants, families, support coordinators, plan managers and health professionals. Self-referrals are welcome.

We're in the process of NDIS registration. Until it's complete, we work with plan-managed and self-managed NDIS participants, and private clients.

We can't take agency (NDIA)-managed plans yet.

We visit across Melbourne and surrounding areas, from Werribee in the west to Ferntree Gully in the east, and down to Frankston.

NDIS number and plan dates
Plan manager details (if plan-managed)
Any recent reports, and the NDIS plan if you have it

What happens next

We usually reply to referrals within 12–24 hours, or within 48 hours if you send it on a Friday or over the weekend. Then we call to talk about goals, funding and timing.
