Privacy policy
Harmony Nursing and Allied Health Pty Ltd (ABN 57 699 327 341) provides occupational therapy. This policy explains how we collect, use, store and share personal and health information. We follow the Australian Privacy Principles in the Privacy Act 1988 (Cth) and the Health Records Act 2001 (Vic).
Last updated: 13 October 2026
What we collect
We collect the information we need to provide your occupational therapy. Depending on your situation, that can include:
- your name, date of birth, address, phone and email;
- your NDIS number and plan details;
- health information, such as your diagnosis, health history, assessment results and session notes;
- details of the people involved in your supports, such as your support coordinator, plan manager, GP, representative and emergency contact;
- invoicing and payment details;
- photos or videos, only if you agree.
Why we collect it
We collect personal and health information to provide and plan your therapy, manage funding and invoices, and meet our legal obligations.
If you choose not to give us information we ask for, we may not be able to accept a referral or provide a service.
How we collect it
We collect information from you or your representative. With your consent, we also collect it from people involved in your care, such as a support coordinator, plan manager or GP who refers you.
Where we keep it
Your records are kept securely in our practice management system, Splose. Only your therapist and authorised Harmony staff can access them.
We keep records for the period the law requires.
AI note-taking
Your therapist may use an AI note-taking tool built into our practice software to draft session notes. We always ask you first, you can say no at any time, and saying no won't affect your service. Your therapist reviews and edits every note.
Who we share it with
We only share relevant information with the people you agree to, such as your GP, support coordinator, plan manager or the NDIA. You can change or withdraw your consent at any time by telling us.
There are times the law requires or allows us to share information without your consent, such as:
- if there is a serious risk to your safety or someone else's;
- if we believe a child is at risk of harm;
- if a court orders us to, or another law requires it.
Where it's safe and lawful to do so, we'll tell you if this happens.
